Intake Information: Once you have confirmed an initial appointment over the phone with me I’ll ask for your email and send you a link (one for each partner in the case of a couple) to your personal, confidential client portal where you can complete intake paperwork, schedule future appointments and receive other correspondence from me. Please contact me if you have any difficulty accessing intake documents.
Arriving at my office: You can access the private parking lot via the alley way on the west side of the office. Please enter through front door on the Flint Street side of the building. Most of the time street parking at the front entrance is available.
Cancellation Policy: I appreciate your courtesy of being aware that I only have a limited number of times to see clients during the week and your appointment time has been reserved exclusively for you. If for any reason you are unable to attend this appointment you must call to cancel a minimum of 48 weekday hours (two full weekdays) before the time of the appointment. If you do not cancel or do not show up you will be charged the full fee for that appointment since it cannot be offered to another client and your insurance cannot be billed.
